All Co-Insurance, Copays, and Deductible Payments must be made at the Time Of Service.

Blue Shield of California

We are In-Network With Blue Shield of California PPO Plans. Including Federal BCBS.

If you have an out of state BCBS plan [Ex: BCBS of Illinois, Premera BCBS] - Network Status can be checked via Blue Card Eligibility. To check network status, contact your insurance plan directly OR call us so we can verify your benefits.

United Healthcare

We are In-Network with most United Healthcare PPO's.

This Includes United Subsidiaries such as UHC Oxford or UMR. Please call to confirm plan network status.

Aetna

We are In-Network with Most Aetna PPO's.

This Includes other plans that may be Using Aetna's Provider Network. Such as Meritain Health.

Cigna

We are In-Network with Most Cigna PPO's.

This Plan may vary on coverage, It is recommended that you call to confirm network status on your specific plan.

Medicare
(Part B)

We are a Participating Provider with Traditional Medicare Plans.

When Scheduling, Please include if you have a Supplement Plan

In-Network Insurances HMO Groups

Though In-Network, If you Have an HMO Plan - You cannot self-refer to a specialist or specialty care. To schedule we must have a Valid Authorization and Referral that has been approved by your insurance on file.

John Muir Health

We are a contracted provider in the John Muir Physician Network.

Disclaimer: If you are a John Muir Employee with an EPO or PPO Plan utilizing the JMH Network, We are in Network and do not require Authorization for care.

Out-of-Network Insurance

If you are using an Out-Of-Network PPO Insurance, Or any Insurance that has Out-Of-Network Coverage. We may still bill your insurance on your behalf.

Uninsured / Discounted Cash Rate

If your Insurance is Out-of-Network with no Out of Network Coverage (Such as Kaiser), Or you are Uninsured or Underinsured. We offer a Discounted Cash Rate. The rate is Based on Time in Per 30 Minute Increments.

For Evaluation or 1 Hour Follow-up Appointments - $160 Per Session

For Half-Hour (30 Minute) Follow-up Appointments - $80 Per Session
